EWB-USA Guidelines: EWB-USA has several Technical Guidelines that have been written by members of the Technical Advisory Committee (TAC).  These guidelines are an invaluable starting point for all EWB-USA projects.  They are viewable by members in the "Resources" section of the "MyEWB" portion of the website.

407 - Professional Chapter Contact Information Spreadsheet for contact information for a professional chapter.

 Section 5: Projects 

500 - Partnering Project Application This form is to be used when a chapter is partnering with another organization on a project (i.e. Habitat for Humanity).  It's just a way to see what our chapters are up to.
501 - New  Project Application - English 

Submit to the TAC for all new projects to be reviewed by the ARC to determine if it will be a viable EWB-USA project. Example Project Application

501B - Continuing Project Application Form This short application is for current EWB-USA chapter projects that are moving on to a new phase of their project.
502 - Chapter Application to Acquire an EWB-USA Project This is to be submitted by a chapter wanting to work on an EWB-USA open project. Only established chapters can apply for open projects.
505 - Site Assessment Form This evaluation tool was created by the Technical Advisory Committee and is an invaluable resource to projects.
506 - Project Expense Form This form is to be used for reimbursements, cash advances and the reconciliation of cash advances for all Project Expenses.
506 - Sample Project Expense Form This is an example of how to fill out the new 506.
507 - Project Report Format Please see the new 507 - Project Design Compendium.
507 - Project Design Compendium This form replaces many of the forms currently required.  It is to be completed continually as the project unfolds.
507 - Project Design Compendium Example This is a sample submission of the 507 - Project Design Compendium form.
507C - Sample Report for 507, Section C This is a sample submission for section C of the 507 - Project Design Compendium form.
509 - Community Health Assessment Information necessary for a baseline health assessment of a community and a reference for determining the benefits of a project. See a Sample Community Health Assessment

512 - One-Page Project Description This is to be submitted for grant applications or if the chapter would like it to be posted on the website to help with donations
Implementation Design Report Example This is an example of an implementation design report submission.


Section 6: Travel 

601 - Travel Readiness Checklist This is a checklist to help chapters prepare for travel.
602 - Travel Tips Presentation This is to be reviewed by each member traveling so that they are aware of practical guidelines of traveling internationally.
603 - Personal Health Checklist This must be filled out by each person traveling and then submitted to the Health Point Person traveling with the project team (Not to EWB-USA)
604 - Recommended Medical Kit Contents Checklist of all of the medical items that should accompany every project trip
605 - Volunteer Waiver Agreement  This document MUST be filled out by each person traveling and submitted to EWB-USA
606 - Emergency Information & Contact Sheet This is to be used as a guideline for information to be provided to all team and family members traveling on the project.
607 - Receipt Form  Print and take on project trips to make receipts when the store or individual is unable to provide a receipt. All EWB-USA reimbursements require a receipt.
608 - Health Insurance Form This document MUST be filled out by each person traveling and submitted to EWB-USA


Section 7: Funding, Financial & Outreach

701 - Grant Policy Overview of EWB-USA grant policy for any chapter, individual, board member or other wishing to submit a grant application.
704 - Chapter Finance Policies  This document provides the policies and procedures for financial management of both University and Professional Chapters.
705 - Chapter Expense Form This form is to be used for reimbursements, cash advances and the reconciliation of cash advances of Chapter Expenses.
705 - Sample Chapter Expense Form This is an example of how to fill out the new 705.
706 - Donor Information Sheet
This spreadsheet is to be used when checks are sent to EWB-USA National for deposit.

802 - ARC Guidelines These are the guidelines that the ARC members use to review project applications
803 - ARC Review Form This is the form used by the Application Review Committee (ARC) to evaluate project applications (501)
806 - Assessment Presentation This presentation was replaced by the 507B - Pre-Assessment Report.  Please see 507- Project Design Compendium.
807 - Implementation Presentation This presentation was replaced by the 507B - Pre-Implementation Report.  Please see 507- Project Design Compendium.
806 - 807 Logistical Checklist Prior to Travel Contains important information of all things required in order for travel to be approved.
